The Senior Outreach Program, which is part of the Unity Health System, provides a variety of
free supportive and mental health services in urban, suburban, and rural areas in Rochester and Monroe County. This program is unique in that all services are brought to the older adult's environment.

This model mental health program delivers service via a mobile outreach team. It is based on successful mental health specialty services using an integrated mental health and primary care model. The Senior Outreach Program actively reaches older persons by collaborating with long-term care services. Mental health assessment, evaluation, and treatment are offered in a variety of settings, including homes, primary care offices, and senior residences. Case management is also used to increase older adults' access to mental health services. The program also provides education about mental health issues for its staff and clients.

All interventions, which are based on evidence-based practices, are focused on high-quality, continuous mental health care for the elderly. The interventions are designed to provide accessible, non-stigmatizing care to elders and to empower older adults to select the most appropriate and comfortable treatment.
